The purpose of documentation is to provide the reader with a path back to your sources.  In principle a reader who questions what you say or wants to pursue your idea further could go back to each and every source you use.

 

Essentials for a book

          author

          title

          translator if necessary

          publication date (if reprint, give original date too)

          page you are referring to

 

Essentials for an article

          author

          title

          publication date

          journal or book in which published

          volume if journal, editor if book

          page you are referring to

 

Essentials for score

          composer

          full title

          publisher

          date of publication

          series, vol, number (if part of series)

          editor (if significant)

 

Essentials for recording, video

title of recording

author (performer, composer)

medium (LP, VHS, DVD etc)

publisher

publisher’s number

date (often hard to find)

 

Essentials for online source

          author

          title

          URL

          access date

 

There are many formats for documentation.  You may use any format you like, as long as you convey the essentials.  Here are some websites that give models for each format:

"Son of citation machine" is a website that does the mechanics for you in any standard format.  Be careful, because it does not always ask for all the information you need, and it does not correct your mistakes.
